Dog Surrender Form

In an effort to provide the highest quality of care to the animals in our shelter, we have a managed intake approach to animals being surrendered into our care.

This means that you must first submit a request to surrender your pet, we will review your application, determine if and when we can best accommodate your request, and then we will reach out to you to provide next steps. This approach ensures we have the appropriate space in our shelter to give your animal the time, attention, and care they deserve.

We recommend giving at least a 30-day notice that you intend to surrender your pet to ensure we can plan appropriately. We cannot accommodate most urgent (less than 3-days’ notice) requests for surrender.

Please be sure to read the Resources for Retention page prior to submitting a form as there are several resources created for you to help you keep your pet happy and healthy at home.

We will not accept any owner surrender animal without first submitting the surrender form.

Note: Submission of this form does not guarantee we will be able to take your dog as our space is limited and the demand is high. Surrender requests from those within Huntington County will take first priority.
